AMY GIORGIO
OCCUPATIONAL THERAPIST & MARKETING  

Amy came to Occupational Therapy later in life, graduating with her Master of Science in Occupational Therapy from the University of South Dakota in 2011. She enjoys working with people and families of all ages, including her work in assisted livings, home health, nursing homes, and pediatrics. She is a certified dementia care practitioner, a certified lymphedema therapist, and certified in LSVT BIG, a program for those with Parkinson’s Disease.

She most recently completed the training as a Certified Living in Place Professional (CLIPP). Prior to becoming an OT, Amy was engaged in mission work in the rural South for five years after earning her Master of Arts in Theological Studies. She then stayed at home for 10 years to raise her four children. OT is a wonderful niche for Amy because it allows her to focus on the physical, emotional, spiritual and mental well-being of those she serves. She and her husband John live in Elk Point, SD.

Megan Struck  

PHYSICAL THERAPIST

Megan graduated from Morningside University in 2016 with a Bachelor’s degree in Biology and Chemistry. She went on to graduate from the University of Iowa with a Doctor of Physical Therapy degree in 2018. Megan has experience working with a wide variety of orthopedic conditions in the outpatient setting as well as inpatient, home health, and skilled nursing facility settings. She has also taken multiple continuing education courses for treatment of pelvic health conditions.

 

In her free time, Megan enjoys spending time outdoors doing activities such as gardening, hiking, running, kayaking, and walking her dog. Megan is currently in the process of moving closer to Sioux City so she can spend time supporting the Morningside Mustangs where her husband is the head tennis coach. She is looking forward to the next chapter of her career with Goodcare AtHome Rehab and the opportunities this will present for herself and the patients of Siouxland! 

John Anzalone  
PHYSICAL THERAPIST  

Dr. John Anzalone has been practicing Physical Therapy since graduating from Creighton University in 2008.  He has specialized in orthopedics with advanced education in manual therapy techniques and differential diagnosis of the musculoskeletal system.  His experience has spanned the entire age spectrum but particularly enjoys working with seniors.  He has extensive experience with rehabilitating patients post orthopedic surgeries such as knee, hip, and shoulder replacements.  He also enjoys working with patients who suffer from generalized weakness and balance issues.  He has worked with many patients who’s diagnosis include but are not limited to Parkinson’s, post CVA/stroke, and other neurologic impairment.  Dr. Anzalone is married and has 4 young boys.  They love spending time together outdoors camping, fishing and hiking.  

Jennifer Leonard  

PHYSICAL THERAPIST ASSISTANT 

Jennifer was born and raised in cozy hometown Holstein, IA. She enjoyed living in a few other places before moving to Waterloo, IA for college, where she intended to settle. However, her plans were changed for the better when she moved to Correctionville, IA to be with him. They are blessed with 3 beautiful kids that are wise beyond their years!  Jenn enjoys being outside where most of extra curricular activities take place. Biking, swimming, yard games, animals, and kayaking.

 

Jennifer has been a PTA since 2013 with a goal to impact lives while also being impacted from others who are wiser and more experienced. Jenn is looking forward to a new chapter in her life with Goodcare AtHome Rehab and meeting all  the wonderful people God has planned for her journey! 

SAMANTHA GRIMSLEY
OCCUPATIONAL THERAPIST

Samantha received her Masters of Science Degree in Occupational Therapy from the University of South Dakota in 2013. She also attended the University of Nebraska, Lincoln where she received her BA in Psychology.

Samantha has worked in skilled nursing for the last 7 years before joining the GoodCare AtHome Rehab Team this year. Samantha loves spending time with her two young kids, Gaines and Ciena and husband Dylan. In her free time, she enjoys working out, laying by the pool, and watching Netflix. She looks forward to serving the aging population in their Sioux City homes.

Jenna Houseman

OCCUPATIONAL THERAPIST

Jenna graduated from Southwest Minnesota State University in 2021 with a Bachelor of Science in exercise science as well as a certificate in professional writing before continuing her education at Nebraska Methodist College where she earned her Master of Occupational Therapy degree in 2023. She has numerous certifications and previous experiences working with this population that will serve her well as part of the Goodcare AtHome Rehab team.

 

In her free time Jenna enjoys hiking with her dog, traveling, going to her high school siblings’ sporting events, and supporting the Briar Cliff Chargers where her fiancé is the offensive line coach for the football team. She looks forward to kicking off her career at GoodCare where she will be able to fulfill her passion for promoting independence within the aging population!

HEATHER HOLBROOK  
PHYSICAL THERAPIST  

Heather received her Doctor of Physical Therapy degree in 2012 from the University of South Dakota.  She is certified in vestibular rehabilitation techniques and has extensive experience with orthopedic conditions, post-operative conditions, generalized weakness, and balance deficits.  During her free time, she enjoys spending time with her husband and 4 young children, extended family and friends.  She is passionate about helping individuals achieve their goals and maintain mobility as they age.
