Amy came to Occupational Therapy later in life, graduating with her Master of Science in Occupational Therapy from the University of South Dakota in 2011. She enjoys working with people and families of all ages, including her work in assisted livings, home health, nursing homes, and pediatrics. She is a certified dementia care practitioner, a certified lymphedema therapist, and certified in LSVT BIG, a program for those with Parkinson’s Disease.

She most recently completed the training as a Certified Living in Place Professional (CLIPP). Prior to becoming an OT, Amy was engaged in mission work in the rural South for five years after earning her Master of Arts in Theological Studies. She then stayed at home for 10 years to raise her four children. OT is a wonderful niche for Amy because it allows her to focus on the physical, emotional, spiritual and mental well-being of those she serves. She and her husband John live in Elk Point, SD.


Gina graduated from Iowa State University in 1993 with a Bachelor of Science degree in Zoology.  She went on to Creighton University where she graduated in 1996 with her Doctorate in Physical Therapy.  Gina has practiced in a variety of therapy settings to include both inpatient and outpatient, acute neuro, and long term care.

She is certified in Parkinson’s LSVT BIG, lymphedema management and has been a clinical trainer for complex disease management.  Gina enjoys baking, reading, exercising and spending time with her friends and family consisting of husband Corey and three children. 


Samantha received her Masters of Science Degree in Occupational Therapy from the University of South Dakota in 2013. She also attended the University of Nebraska, Lincoln where she received her BA in Psychology.

Samantha has worked in skilled nursing for the last 7 years before joining the GoodCare AtHome Rehab Team this year. Samantha loves spending time with her two young kids, Gaines and Ciena and husband Dylan. In her free time, she enjoys working out, laying by the pool, and watching Netflix. She looks forward to serving the aging population in their Sioux City homes. 

Ann Madsen
Physical Therapist Assistant

Ann graduated from the physical therapist assistant program at WITCC in Sioux City, IA. Ann loves working with all ages and abilities.  She has worked in skilled care, acute care, hospital setting, and home health.  Ann thrives on seeing the success of others in improving their physical well-being.  Being active herself, she enjoys boating, working out and playing with her 2 grandchildren.

Brooke Hindman
Occupational Therapist

Brooke graduated from the University of South Dakota with a Bachelor’s degree in Psychology in 2010 and her Masters of Science in Occupational Therapy in 2013.  She lives in Sioux City, Iowa with her husband, Thomas, and her daughter, Rowan, and son, Bodhi.  Brooke has worked with individuals across the lifespan, having worked in pediatrics and geriatric settings.  Brooke is a CBIST, a Certified Brain Injury Specialist Trainer.  She is passionate about treating those with brain injuries.  Brooke enjoys spending time with her family and her dog, as well as spending time outside.



Heather received her Doctor of Physical Therapy degree in 2012 from the University of South Dakota.  She is certified in vestibular rehabilitation techniques and has extensive experience with orthopedic conditions, post-operative conditions, generalized weakness, and balance deficits.  During her free time, she enjoys spending time with her husband and 4 young children, extended family and friends.  She is passionate about helping individuals achieve their goals and maintain mobility as they age.



Michelle graduated with her Masters degree in 2008 from University of Mary. In 2011, she earned her doctorate from Rocky Mountain University. For past 7 years, Michelle has focused in serving long term care residents. She also has experience in pediatrics, acute care, outpatient and home health. Michelle became an occupational therapist as she enjoys the diversity of the OT world and the many populations we can serve.


Michelle lives in Sgt Bluff with her husband, Dan, son Broxton and dog Bear. In her free time, she enjoy working out, gardening and spending time with family and friends. Michelle is looking forward to collaborating with the facilities and serving the community of Siouxland.

Kelsey Peterson


Kelsey graduated in 2014 from Wayne State College with a Bachelor’s degree in Exercise Science. In 2017, she graduated with her Doctorate in Occupational Therapy from the University of South Dakota. She has worked in many settings since beginning her career, including inpatient, outpatient, home health, and skilled nursing facilities. Kelsey enjoys being an OT because she is able to help individuals remain safe and independent as they age or recover from injury. She looks forward to providing quality OT services to the residents of Northwest Iowa, where she resides with her husband, Shane.



Jenna graduated in 2010 from the University of Wisconsin-Eau Claire with a B.S. in Communication Sciences and Disorders with a minor in Theatre Arts; then in 2014 graduated from the University of South Dakota with an M.A. in Speech-Language Pathology.

 For the past 6.5 years Jenna has been primarily serving residents in area skilled nursing and assisted living facilities, but also has experience in early intervention/coaching and adult outpatient. Interests include cognitive-communicative disorders, orofacial myofunctional disorders, and dementia care.

Jenna lives in Moville with her husband Phil and three children, sons Landon and Cameron, and daughter Claire. A die hard Packers fan and cheese loving Wisconsin native, Jenna enjoys reading, cooking, and crafting in her spare time. Jenna is looking forward to serving Siouxland residents to help them reach their highest potential for a rich and fulfilling life.  

Jennifer Leonard


Jennifer was born and raised in cozy hometown Holstein, IA. She enjoyed living in a few other places before moving to Waterloo, IA for college, where she intended to settle. However, her plans were changed for the better when she moved to Correctionville, IA to be with him. They are blessed with 3 beautiful kids that are wise beyond their years!  Jenn enjoys being outside where most of extra curricular activities take place. Biking, swimming, yard games, animals, and kayaking.


Jennifer has been a PTA since 2013 with a goal to impact lives while also being impacted from others who are wiser and more experienced. Jenn is looking forward to a new chapter in her life with Goodcare AtHome Rehab and meeting all  the wonderful people God has planned for her journey! 

Nikita Kleyer


Nikita graduated from Augustana in 2010 with a Bachelor of Science in Communication Disorders and in 2012 from St. Ambrose University with a Master of Science in Speech Language Pathology. 

Nikita has worked with patients of all ages in a variety of settings including, but not limited to, feeding clinics, schools, home health, acute and outpatient care, as well as skilled nursing and assisted living facilities. She currently works primarily in a school setting, but has consistently worked in skilled nursing and assisted living facilities over the last 7 years. Nikita enjoys the depth of my position and the variety of people it allows me to serve. 


Nikita lives on a farm outside of Orange City with her husband Matt. They currently have 4 children, both adopted and biological, but as a foster family, we are blessed with new members often. In addition to being a speech pathologist, Nikita is the founder and executive director of the Awakening Grace Foundation. In her spare time, she enjoys projects in which she can create or build, redesigning and redecorating her home, reading, cooking, and dreaming up new ways to support underserved populations. Nikita is looking forward to working with the people of Siouxland and getting the opportunity to meet their needs.
