Physical Therapy

  • Recent Falls
  • C/o pain
  • Balance concerns: standing or sitting
  • Fear of falling or walking

  • Improper user of assistive device
  • Uses objects for support
  • Muscle rigidity
  • Difficulty standing or transferring

Occupational Therapy

  • Recent Falls

  • Increased difficulty with self-care tasks
  • Increased spillage of food when eating or difficulty cutting or scooping food
  • Increase in overall dependence with daily tasks, safety of tasks.
  • Decline in cognition: memory, safety, judgement, organization
  • Behaviors due to cognitive decline
  • Poor coordination
  • Weakness
  • Balance concerns: standing or sitting
  • Decreased endurance for outings or community events
  • Decreased participation in activities

Speech Therapy

  • Eating/swallowing difficulty
  • Weight loss
  • Choking
  • Change in diet consistency
  • Speech difficult to understand
  • Poor voice control
  • Shortness of breathe limits conversation
  • Decline in memory
  • Gets lost inside facility
  • Difficulty following directions
  • Difficulty hearing conversations
  • Reports new difficulties with managing checkbook, scheduling activities, remembering appointments
  • Loses possessions or new decline in recall of important information.
